ECHS Medicine Bill claim karne ke liye kaun-kaun se documents chahiye?

Generally aapko ye documents submit karne hote hain:

  • Original Medicine Bills
  • Doctor Prescription
  • ECHS Card Copy
  • Cheque Book
  • Contingent Bill (Bill Claim karte samay generate hota hai)

Answer:
Referral ki validity treatment ke type ke hisab se alag ho sakti hai. Isliye hospital jane se pehle referral ki validity zarur check karein. 

1. OPD Referral (Outpatient Department)

  • OPD referral ki validity 3 months hoti hai.

  • Is 3 months ke andar aap maximum 6 OPD sessions ka benefit le sakte hain.

  • Agar 6 sessions complete ho jate hain ya 3 months ki validity khatam ho jati hai (jo pehle ho), to aage treatment continue karne ke liye naya referral lena hoga.

  • Har visit par referral ki copy aur ECHS Smart Card saath lekar jayein.

2. Diagnostic Test Referral

  • Diagnostic tests (jaise Blood Test, X-Ray, CT Scan, MRI, Ultrasound, ECG, etc.) ke liye diya gaya referral 3 months tak valid hota hai.

  • Is validity period ke andar har prescribed test sirf ek hi baar karwaya ja sakta hai.

  • Agar test 3 months ke andar nahi karwaya gaya ya doctor ko test dobara karwana ho, to fresh referral ki zarurat padegi.

3. IPD Referral (Indoor Patient Department / Hospital Admission)

  • IPD referral bhi 3 months tak valid hota hai.

  • Is referral ke basis par sirf ek baar hospital admission (One Admission Session) ki permission hoti hai.

  • Agar patient ko dobara admission ki zarurat padti hai ya referral ki validity expire ho jati hai, to naya IPD referral issue karwana hoga.

Important Note

  • Hospital visit se pehle referral ki issue date aur validity zarur check karein.

  • Referral expire hone ke baad hospital treatment ya diagnostic services provide nahi karega.

  • Kisi bhi confusion ki situation mein apne ECHS Polyclinic se contact karke fresh referral issue karwa sakte hain.


 

Answer:

Agar patient ki condition life-threatening ho to:

  • Nearest empanelled ya kisi bhi non empanelled hospital mein admission karwa sakte hai.
  • ECHS emergency admission rule  ke anusar, aap emergency me kisi bhi non empanelled hospital me admission kara sakte hai. Bas aapko 48 hrs ke ander apne ECHS polyclinic me information deni hoti hai.
  •  Hospital se discharge hone ke baad apko sabhi documents ke sath apne ECHS Polyclinic me jakar reimbursement ka process karna hota hai. 

Answer:
Ji haan. Agar genuine medical emergency ho to patient nearest empanelled ya suitable hospital mein treatment le sakta hai. Baad mein ECHS rules ke according reimbursement claim submit kiya ja sakta hai, subject to eligibility and document verification.
