ECHS Card DOB Correction Process for Parents | Father Mother Date of Birth Update Guide 2026

Sirf ye 5 simple steps follow karke apne ECHS Card me father aur mother ki Date of Birth Correction kar sakte hain:

Step-1

Affidavit banwaye – Parents ke DOB correction ke liye legal affidavit banbaye, jiska format niche diya hua hai.

Step-2

ZSB through documents bhejwaye – Apne Zila Sainik Board (ZSB) ke through documents ko apne Centre me bhejwaye.

Step-3

Part-II ko Apne discharge Book me entery karwaye:

Jab apka Part-II hokar speed post ya mail Id me aa jata hai, tou isko apne ZSB me jakar discharge Book me entery karwa le.

Step-4

ECHS Card Reapply Kare – Echs ke official site me jakar, apne parents ke echs card ko block kare aur Reapply Kare.

Step-5

Temporary slip signature karwaye – Apply karne ke baad temporary slip ko download Karen aur form 26 AS, aadhar, pan, discharge Book aur PPO ke sath apne parents polyclinic me jakar documents ko signature karwaye, taki treatment start ho sake.
